I acknowledge that I have read and understand the following: 1) The LGBTQ Center shall conduct a background check, which may include review of my criminal history, sex offender status, and other public records as required for volunteer service. 2) Law enforcement agencies, courts, and other relevant entities shall release information to The LGBTQ Center for this purpose. 3) All information obtained shall be kept confidential and used solely to determine my eligibility to serve as a volunteer. 4) Under the Fair Credit Reporting Act, my consent is required before any such screening is performed.
